What is in the leave request form
The file, plyntum-leave-request-form.docx, is a one-page form with five blocks:
- Employee: name, position, manager, date of the request.
- Leave requested: type (annual paid, unpaid, sick, parental, other), first and last day, working days, first day back, and an optional reason.
- Cover while away: each client, project or recurring task, who covers it, and whether the handover is done, plus how the person can be reached, if at all.
- Decision: approved, approved with changes, or declined, the changed dates or the reason, and the manager’s signature.
- For the records: leave days before this request, days taken, days remaining, and who recorded it.
The document opens in Word, Google Docs and Pages. A grey note at the top explains how to use it; delete it before printing.
How to use it in a small team
- The employee fills in blocks 1 to 3 and arranges the cover before asking, not after. A request that already names who covers each client is approved faster.
- The manager decides on the same form and writes the reason when the answer is “with changes” or “no”, so the conversation does not have to be repeated later.
- Whoever keeps the records updates the days remaining. In a company of five that is the founder; keep one folder per person per year.
- Put the dates in the shared calendar and tell the clients the person covers, in writing, a week before.
Two copies: one with the employee, one in the folder. If you run leave through a tool instead of paper, the same five blocks are the fields that tool should have.
Leave entitlements, and what this form does not do
The form records a request and a decision. It does not decide how many days a person is entitled to: that comes from the employment contract, your company policy and the labour law that applies to the employment. In Georgia the Labour Code sets a minimum of 24 working days of paid leave a year, with further rules for unpaid, sick and parental leave; check the current text and your contracts before you rely on a number, and ask a lawyer for anything that is not routine. Keep the form the same for everyone. A request that is approved by message for one person and by form for another is the kind of inconsistency that becomes a dispute later.
